ALL NEW / INCOMING STUDENT-ATHLETES are required to meet with a member of the athletic training staff on their team's designated date and time, and go over their medical history form. In addition to this, the athlete will meet with the team physician for a physical screening. The final decision on physical qualifications or reasons for rejection is the responsibility of Gannon University's team physician and athletic trainers.
No outside physicals will be accepted.
Prior to this meeting, athletes must fill out the forms listed below. All forms must be on file in the athletic training department prior to the athlete's participation in any team practice/activity. They can be printed out and mailed to the address listed below the links.
Simply mailing the forms does not exclude any athlete from meeting with a member of the athletic training staff. Atheletes will not be allowed to participate in any team practice/activity until a member of the athletic training staff has cleared them to do so.

The deadline for all medical forms listed above (including photocopy of front and back of insurance card) is
July 15, 2013. The student-athlete's on-campus pre-participation physical examination
will not be completed by the athletic training staff until all forms are filled out completely and returned by the
July 15th deadline.
